Download PDF
To Report an Occurrence Offline, can can download the PDF form that corresponds to the webform you want to use by clicking on the 'Download PDF' button.
Open it with 'Adobe Reader', fill the required information and save it in your computer.
Upload PDF
Click on the 'Upload PDF' button on the right pane. You can upload the PDF form by 'Drag and drop' or by selecting itfrom your computer
You can see if the document has been uploaded correctly or not. And you can delete it by clicking on the 'x' button
Additional documents
You are able to attach more documents or click on the 'Continue' button to go to the next step
Personal details
The ‘Personal Details’ section can be filled in, optionally, by unregistered users:

This section will appear only if you are not logged in.
Submit
Click on the 'Submit' button located on the bottom of the page. The record will now be saved in the data base as Original Report.
Automatic confirmation email will be sent, only if an email address has been provided (unregistered user) or if you are logged in.
Cancel
When reporting an occurrence, the process can be cancelled at any point by clicking on the 'Cancel' button located on the bottom of the page.
